The National Transportation Safety Board is reviewing the door plug blowout aboard an Alaska Airlines Boeing 737 Jet in 2024.

A preliminary report found that four bolts meant to secure the door plug were missing when the incident occurred.

The investigation has already revealed that bolts were never replaced after the door plug was removed during a repair.
